In addition to the Membership Application, Transmission-Owning Members are also required to fully execute and submit the followings:

1. A signature page to the Agreement of Transmission Facilities Owners to Organize the Midwest Independent Transmission System Operator, Inc., A Delaware Non-Stock Corporation ("Midwest ISO Agreement"), FERC Electric Tariff, First Revised Rate Schedule No. 1.

2. A signature page to the Supplemental Agreement, which is a multi-party contract between the Midwest ISO, International Transmission Company (“ITC”) and each of the Midwest ISO Transmission Owners ("Supplemental Agreement").  The Supplemental Agreement amends the Appendix I Agreement between ITC and the Midwest ISO.

3. A signature page to the Agency Agreement (pursuant to Appendix G of the Midwest ISO Agreement), which authorizes the Midwest ISO to act as the Member's agent in the performance of Tariff administrative duties with regard to "Non-transferred Transmission Facilities."

4. A signature page to the Amended Balancing Authority Agreement between the Midwest ISO and the Midwest ISO Local Balancing Authorities as a result of negotiations regarding the establishment and implementation of the Midwest ISO Ancillary Services Markets.
